All paperwork must be filled out completely and signed with a partial payment received prior to any session or services.  Full payment is due at time of any session or service.  You must cancel a minimum of twenty-four (24) hours prior to your appointment to receive a full refund if paying in advance. A mileage fee will be included for all house calls outside my 5 mile radius.
